Do you take my insurance?
No - Trailblaze is an out-of-network provider- this is in the best interest of our patients. This model allows us to:
Provide one-on-one care with your Doctor of Physical Therapy, Katharine McWilliams, PT, DPT, CSCS
Implement an individualized plan without insurance restrictions on visits
Create maximal value in every session
Have flexibility in our plan to fit into your busy lifestyle
Please note you can request a SUPERBILL and submit it for reimbursement through your out-of-network benefits. We also accept payment through your FSA and HSA. Trailblaze PT+W is more than happy to help you through this process or with any further questions.
Do I need a prescription from a Medical Doctor?
Nope! Arizona is an unrestricted direct access state. What is direct access? A person’s ability to receive physical therapy treatment WITHOUT a doctor’s prescription/recommendation. You sprain your ankle Saturday? You can schedule a visit with a physical therapist to start working on this issue immediately. For nearly all muscle, joint, tendon, ligament, and bone issues, early treatment can improve long-term outcomes. This means you should seek treatment sooner rather than later by a physical therapist.
**NOTE: if you DO have a physical therapy prescription from your primary care doctor, orthopedist, neurologist, etc, it is your right as a patient to choose where and with whom you receive treatment.
What should I expect during my evaluation?
A Trailblaze PT+W Evaluation has four phases:
Comprehensive review of past medical history and detailed report of current issue/injury
Thorough examination of your current status- including range of motion, strength, functional movement, stability, joint mobility, balance, etc.
A discussion of findings and your recommended plan based on these findings
Start treatment! Exercise prescription, manual treatment, or both will begin on your first day as well
I saw your “Specialties,” does Trailblaze PT+W treat other body parts/issues as well?
